34 year old Kristin Renee Czyz of Calgary died due to a parachute failure after jumping from Perrine Bridge in Twin Falls, Idaho on May 13 according to Global News Canada.

Investigators found a hole in the pilot chute that is used to deploy the main chute from its pack, as well as a broken zipper on her harness.  Czyz had borrowed the pack from a local shop. It was her 5th jump from the bridge over the Snake River that day.

The Twin Falls County Sheriff’s Office has determined the death to have been an accident.
